Isoniazid Tablet for The Prevention of Various Types of Tuberculosis
Isoniazid tablets

Generic name isoniazid tablet

The main ingredient of this product is isoniazid.

1. Isoniazid is indicated in combination with other antituberculous agents for the treatment of all types of tuberculosis, including tuberculous meningitis and other mycobacterial infections.

2. Isoniazid alone is suitable for the prevention of various types of tuberculosis:

(1) Family members or close contacts of newly diagnosed TB patients.

(2) patients with strong positive PPD test and chest X-ray examination consistent with non-progressive tuberculosis, negative sputum bacteria, who have not received regular anti-tuberculosis treatment in the past.

(3) Patients undergoing immunosuppressive or long-term hormone therapy, certain blood diseases or reticuloendotheliosis (e.g., leukaemia, Hodgkin's disease), diabetes, uremia, silicosis or gastrectomy, and who have tested positive for tuberculin pure protein derivatives.

(4) Patients under 35 years of age who tested positive for tuberculin pure protein derivatives.

(5) known or suspected HIV-infected persons who have tested positive for tuberculin pure protein derivatives, or who have had close contact with patients with active tuberculosis.

Adverse reactions 1. The incidence of gait instability or numbness, tingling, burning or finger pain (peripheral neuritis).

2. Dark urine, yellow eye or skin (hepatotoxicity, increased incidence of hepatotoxicity in patients over 35 years of age).

3. Poor appetite, unusual weakness or weakness, nausea or vomiting (a prodrome of hepatotoxicity).

4. The incidence of very few people have blurred vision or vision loss, with or without ophthalmophilia (optic neuritis).

5. Fever, rash, cytopenia and male breast development.

5. This product may occasionally cause convulsions due to neurotoxicity.

People with abnormal liver function, mental illness and epilepsy are contraindicated.

1. Cross-allergic reaction. People who are allergic to ethylthiisonicotinamide, pyrazinamide, niacin or other chemically related drugs may also be allergic to this product.

2. Interference to the diagnosis: the determination of urine sugar by copper sulfate method can show false positive reaction, but it does not affect the result of enzymatic determination. Isoniazid can increase the determination of serum bilirubin, alanine aminotransferase and aspartate aminotransferase.

3. Patients with a history of psychosis, epilepsy and severe renal impairment should be used with caution.

4. If optic nerve inflammation appears during the course of treatment, eye examination should be conducted immediately and regular review should be carried out.

5 isoniazid poisoning can be used large dose of vitamin B6.

1. This product may cross the placenta, resulting in fetal blood concentration higher than that of the mother. Animal experiments have confirmed that isoniazid can cause stillbirth, but it has not been proven in humans. The advantages and disadvantages of isoniazid should be fully weighed when used by pregnant women. The effects of isoniazid on the fetus in combination with other drugs have not been clarified. In addition, the adverse reactions should be closely observed when the drug is used in neonates.

2. The concentration of isoniazid in milk can reach 12mg/L, which is similar to that of blood drug; Although not proven to be a problem in humans, the pros and cons of use during lactation should be fully weighed. If medication should stop breastfeeding.

Children's medicine should be used strictly according to children's usage and dosage.

The incidence of hepatitis caused by this product is higher in patients over 50 years old.

1. Daily drinking when taking isoniazid is likely to cause liver toxicity induced by this product and accelerate the metabolism of isoniazid. Therefore, the dose of isoniazid should be adjusted and the signs of hepatotoxicity should be closely observed. Patients should be advised to avoid alcohol while taking the medication.

2. Acid containing aluminum can delay and reduce the absorption of isoniazid after oral administration and reduce the blood concentration of the drug, so avoid taking both at the same time or taking isoniazid at least 1 hour before oral administration.

3. When anticoagulants (such as coumarin or indendione derivatives) are used together with isoniazid, the anticoagulation effect is enhanced by inhibiting the enzyme metabolism of anticoagulants.

4. Concurrent administration of cycloserine may increase central nervous system adverse reactions (e.g., dizziness or drowsiness). Dosage should be adjusted and signs of central nervous system toxicity closely observed, especially in patients performing tasks requiring high sensitivity.

5. The combination of rifampicin and isoniazid may increase the risk of hepatotoxicity, especially in patients with existing liver function damage or rapid acetylation of isoniazid. Therefore, close follow-up should be conducted for signs of hepatotoxicity during the first 3 months of treatment.

6. Isoniazid is an antagonist of vitamin B6. It can increase the output of vitamin B6 through the kidney, which may lead to peripheral neuritis.

7. When combined with adrenal corticosteroids (especially prednisolone), the metabolism and excrement of isoniazid in the liver can be increased, resulting in a decrease in the plasma concentration of isoniazid and thus affecting the efficacy, especially in rapid acetylation, so the dose should be adjusted appropriately.

8. When combined with alfentanil, isoniazid is an inhibitor of liver drug enzymes, which can prolong the action of alfentanil; Combined with Disulfiram, it can enhance the central nervous system, resulting in vertigo, discoordination, irritability, insomnia, etc. Combination with enflurane increases the formation of inorganic fluorometabolites with nephrotoxicity.

9. The combination with ethylthioisomide or other antituberculous drugs may aggravate the adverse reactions of the latter two drugs. Combination with other hepatotoxic agents may increase hepatotoxicity and should be avoided as far as possible.

10. Isoniazid should not be used in combination with ketoconazole or miconazole because it may reduce the blood concentrations of the latter two drugs.

11. When used with phenytoin sodium or aminophylline, the metabolism of phenytoin sodium or aminophylline in the liver can be inhibited, resulting in increased plasma concentration of phenytoin sodium or aminophylline. Therefore, the dose of phenytoin sodium or aminophylline should be adjusted appropriately when isoniazid is used together with the two.

12. When combined with acetaminophen, isoniazid can induce isohepatocyte cytochrome P450, resulting in an increase in the amount of toxic metabolites formed by isoniazid, which can increase hepatotoxicity and renal toxicity.

13. When used concurrently with carbamazepine, isoniazid can inhibit its metabolism, increase the blood concentration of carbamazepine, and cause toxic reactions; Carbamazepine can induce the microsomal metabolism of isoniazid and increase the formation of intermediate metabolites with hepatotoxicity.

14. This product should not be used together with other neurotoxic drugs to avoid increasing neurotoxicity.

Drug overdoses were not tested and there were no references.

Drug toxicology This product is a synthetic antibacterial agent with bactericidal action. This product is only effective against mycobacteria, mainly those in the growth and reproduction period. Its mechanism of action has not been elucidated, but it may inhibit the synthesis of mycolicacid in sensitive bacteria and rupture the cell wall.

Pharmacokinetics 1. This product is rapidly absorbed from the gastrointestinal tract after oral administration and distributed in tissues and body fluids throughout the body, including cerebrospinal fluid, pleural fluid, ascites, skin, muscle, milk and caseous tissues. And can cross the placental barrier. The protein binding rate was only 0 ~ 10%. The blood drug concentration can reach the peak after 1 ~ 2 hours of oral administration, but after 4 ~ 6 hours, the blood drug concentration varies according to the speed of acetylation of patients. In rapid acetylation, t1/2 is 0.5 ~ 1.6 hours, in slow acetylation, 2 ~ 5 hours, and may be prolonged in patients with liver and kidney damage. Metabolism is mainly acetylated in the liver to produce inactive metabolites, some of which have hepatotoxicity.

2. The rate of acetylation is determined by genetics. The liver is deficient in n-acetyltransferase in slow acetylation, and unacetylated isoniazid may be partially bound. This product is mainly excreted by kidney (about 70%), and is discharged within 24 hours, mostly inactive metabolites. In the rapid acetylation, 93% of the acetylation was excreted in urine, and 63% of the slow acetylation was excreted.

3. 7% of isoniazid was free or bound in the urine of the rapid acetylation group, while 37% of the slow acetylation group. This product is easy to pass the blood-brain barrier, can also be discharged from milk, small amounts of saliva, sputum and feces. Considerable amounts of isoniazid can be cleared by hemodialysis and peritoneal dialysis.
