Clinical Details
Exfoliative Cytology
The exfoliating cells from various body regions, especially the surface of luminal organ, are stained and observed under microscopes, ccordingly the diagnosis is made. This kind of laboratory medical science is called as exfoliative cytology, also as clinical cytology or cellular pathology.
The Source of Exfoliating Cells
Spontaneous discharge (sputum, urine, nipple discharge etc.) Cavity aspiration (pleural effusion,abdominal dropsy,aquae pericardii, brain effusion) Needle aspiration (soft tissue mass, mammary gland, lymphonodus, thyroid gland etc.)
Morphological Features of Malignant Cells
Caryon atypia: caryons become big and are malformed, the stained caryones are darken and crude, the proportion of caryon & cytoplasm is abnormal, the plasmosomes increase in size and quantity, mitotic figures increase, tumor giant cells and bare nucleus appear.
Cytoplasm atypia: rich highly differentiating malignant cell cytoplasm; little poorly differentiated one.
Cell distribution: without polarity and in chaos.
Slide background :with lots of neutrophile granulocytes, lymphocytes, phagocytes, erythrocytes etc.
Conventional Cytology Diagnosis Technique
Conventional cytology diagnosis technique plays positive role in prior screening and diagnosis. However the examination method is quite simple, it's difficult to collect the cell specimens fully. The manual slide processing has large randomness which causes cells multi-layer overlap and morphological variation. The method has limitations and uncertainty and it's possible that the false negative happens (it's reported that there are 2% to 5% false negative from many cervical slides), some patients with cervical carcinoma missed diagnosis. The false negative causes cervical carcinoma patients miss the chance of early treatment and suffer great misery.
Liquid-based Thin-layer Slide Technique
Non-diagnostic debris is removed through special techniques. The clear thin-layer slide is prepared and easy to be observed and accuracy of diagnosis is improved obviously. It's classified into two types, membrane type (TCT) and sedimentation type (LCT), according to difference of working principles.
TCT Operation Procedures (Membrane Type)
1. Cervical brush stirs in little bottle about tens of seconds the exfoliating cells are washed into cell preserve solution.
2. The non-diagnostic debris is filtered by high precision membrane.
3. Thin-layer cells with diameter 20mm are made on defined position of the glass slide.
4. Fix it with 95% ethanol, pathologists evaluate it under microscopes after staining and sealing.
5. Analyze it with TBS diagnosis analysis system and print results finally.
