Febusostat, a new generation of Xanthine oxidase inhibitor developed by Japan's Imperial Pharmaceutical Company, is clinically used to treat hyperuricemia (gout) in a structure very different from the Xanthine oxidase inhibitor drugs developed 40 years ago, it is a novel and highly effective non purine Xanthine oxidase selective inhibitor. Xanthine oxidase is a key enzyme that promotes uric acid production. Febusutan reduces uric acid levels in the blood of patients with Hyperuricemia gout. Clinical studies have shown that Febusutan is safe and effective. It is metabolized through the liver and does not depend on kidney excretion, therefore, no reduction in dosage is required for patients with moderate to severe hepatic and renal insufficiency. The dosage is once a day, 40 mg or 80 mg at a time, and is not recommended for Hyperuricemia gout patients. In February 2009, the US Food and Drug Administration (FDA) approved Febusutan for the long-term treatment of patients with Gout Hyperuricemia.

Function and Application
GOUT is a group of Heterogeneous Diseases, which is caused by hereditary or acquired decrease of uric acid excretion and disturbance of purine metabolism. As a result of excessive uric acid produced in the body and kidney clearance ability to reduce the accumulation of uric acid in the body, resulting in urate crystals in joints and organs deposition. Therefore, gout is usually treated by promoting uric acid excretion and inhibition of uric acid production, and appropriate measures to improve the symptoms. The production of uric acid in the body is related to the metabolism of purines. In the final step of purine metabolism, Hypoxanthine produces xanthine under the action of Xanthine Oxidoreductase, which in turn produces uric acid, inhibition of the enzyme activity can effectively reduce the production of uric acid. The new generation of non-purine Xanthine oxidase inhibitor, which is currently being developed in the world, reduces uric acid synthesis in the body and reduces uric acid concentration by acting on the oxidase in a highly selective manner, to effectively treat the ventilation disease. For 30 years, allopurinol has been the only drug used to inhibit uric acid production, and has been widely used as the gold standard treatment for Gout, achieving remarkable results in the treatment of Gout. Compared with allopurine, Febusutent had significant advantages: (1) ALLOPURINOL inhibited XOR of reduced type, but not busutent inhibited XOR of both oxidized and reduced type, (2) due to the Purine analogue of the ALLOPURINOL, there is an inevitable influence on the activities of other enzymes involved in purine and Pyridine metabolism. Therefore, repeated large doses of allopurinol are needed to maintain high levels of the drug. As a result, serious and even fatal adverse reactions resulted from the accumulation of drugs. Non-busuotan is a non-purine XOR inhibitor, so it has better safety.
