Common name dipyridamole tablet
Main ingredients The main ingredient of this product is dipyridamole.
Indications This product is mainly used for anti-platelet aggregation, for the prevention of thrombosis.
Adverse Reactions 1. Adverse reactions were light and short at the therapeutic dose, and the initial side effects were mostly disappeared after long-term administration.
2. Common adverse reactions include dizziness, headache, vomiting, diarrhea, flushing, rash and pruritus, and rare angina pectoris and liver insufficiency.
3. Adverse reactions that persist or can not be tolerated are rare and can be eliminated after drug withdrawal.
4. In clinical reports after marketing, rare adverse reactions include laryngeal edema, fatigue, discomfort, myalgia, arthritis, nausea, dyspepsia, paresthesia, hepatitis, baldness, cholelithiasis, palpitations and tachycardia.
It is contraindicated for allergic patients.
Precautions When this product is combined with anticoagulants, antiplatelet aggregation agents and suppositories, attention should be paid to bleeding tendency.
Pregnant women and lactating women have not found teratogenic effects of this product. In pregnant women, it is limited to those with definite indications. This product is discharged into milk, so it should be used with caution.
Drug use in children is unclear.
Drug use for the elderly is unclear.
1. Synergistic action with aspirin. When combined with aspirin, the dose can be reduced to 100-200mg a day.
2. There is no increase or increase in bleeding when this product is used together with dicoumarin anticoagulant.
Laboratory tests for drug overdoses may include abnormal electroencephalography, altered acid-base balance (respiratory alkalosis and metabolic acidosis), hypoglycemia or hyperglycemia, ketonuria, hyposnatremia, hypokalemia, and proteinuria.
Drug toxicology 1. Has the antithrombotic effect. Dipyridamole inhibited platelet release at high concentration (50mg/ mL). The mechanism of action may be:
(1) Adenosine uptake by platelets, epithelial cells, and red blood cells was inhibited in a dose-dependent manner at therapeutic concentrations (0.5-1.9 mg/ dL). As local adenosine concentration increases, it acts on the A2 receptor of platelets, stimulating adenylate cyclase and increasing intracyclic adenosine phosphate (cAMP) in platelets. Through this pathway, platelet aggregation induced by stimulation such as platelet activating factor (PAF), collagen and adenosine diphosphate (ADP) is inhibited.
(2) Inhibition of phosphodiesterase (PDE) in various tissues. Therapeutic concentration inhibited cGMP-PDE and had a weak inhibitory effect on cAMP-PDE, which resulted in increased cGMP concentration induced by enhancement of endothelial dilation factor (EDRF).
(3) Inhibition of thrombalkane A2 (TXA2) formation, TXA2 is a powerful agonist of platelet activity.
(4) Enhance the effect of endogenous PGI2.
2. Dipyridamole has dilating effect on blood vessels. Transduodenal administration of dipyridamole at 0.5 to 4.0mg/kg produced dose-dependent decreases in systemic circulation and coronary resistance, as well as a decrease in systemic blood pressure and an increase in coronary blood flow. The drug takes effect in 24 minutes after administration and lasts for about 3 hours. The same hemodynamic effects were observed in humans. However, acute intravenous administration can reduce local myocardial perfusion in the distal end of the stenosed coronary artery.
3. There was no significant carcinogenic effect of dipyridamole at 8, 25, and 75mg/kg (1, 3.1, and 9.4 times the maximum daily recommended human dose) in oral trials in mice at 111 weeks and rats at 128 to 142 weeks.
4. The mutagenic test was negative.
5. Reproductive experiments in rats with dipyridamole at 60 times the maximum daily recommended human dose showed no evidence of reproductive impairment. However, at 115 times the maximum daily recommended human dose, luteal number was significantly reduced and live embryo implantation was reduced. Tests in mice, rats, and rabbits showed no evidence of fetal damage with dipyridamole.
The oral LD50 of mice was 2150mg/kg. A single oral lethal dose was 6000mg/kg in rats and 350mg/kg in dogs.
Pharmacokinetics Oral absorption is rapid, with a mean peak concentration of about 75 minutes and a plasma half-life of 2-3 hours. High binding rate with plasma protein. It is metabolized in the liver, bound to glucoside acids, and excreted from bile.
