Tacrolimus is used on the skin to treat a skin condition called eczema (atopic dermatitis) in patients who have not responded well to (or should not use) other eczema medications.
Eczema is an allergic-type condition that causes red, irritated, and itchy skin. This drug works by weakening the skin's defense (immune) system, thereby decreasing the allergic reaction and relieving the eczema. Tacrolimus belongs to a class of drugs known as topical calcineurin inhibitors (TCIs).
Tacrolimus  is an immunosuppressive drug whose main use is after organ transplant to reduce the activity of the patient's immune system and so the risk of organ rejection. It is also used in a topical preparation in the treatment of severe atopic dermatitis, severe refractory uveitis after bone marrow transplants, and the skin condition vitiligo.
Tacrolimus is chemically known as a macrolide. It reduces peptidyl-prolyl isomerase activity by binding to the immunophilin FKBP-12 creating a new complex. This FKBP12-FK506 complex inhibits calcineurin which inhibits T-lymphocyte signal transduction and IL-2 transcription.

What is tacrolimus used for?
Tacrolimus is an immunosuppressant. It works by decreasing your body's immune system. Tacrolimus topical (for the skin) is used to treat severe atopic dermatitis (eczema). Tacrolimus may also be used for purposes not listed in this medication guide.
How does tacrolimus affect the kidneys?
Tacrolimus is a medicine that lowers the body's natural immunity. The immune system is thought to be involved in a disorder which causes the kidneys to leak protein into the urine (Nephrotic Syndrome). It is widely used to prevent rejection of transplanted organs, such as the kidney, heart, and liver.
