Description | Tacrolimus (also FK-506 or fujimycin, trade names Prograf, Advagraf, Protopic) is animmunosuppressive drug used mainly after allogeneic organ transplant to lower the risk of organ rejection. It achieves this by inhibiting the production of interleukin-2, a molecule that promotes the development and proliferation of T cells, which are vital to the body's learned (or adaptive) immune response. Tacrolimus is also used in the treatment of other T cell-mediated diseases such as eczema (for which it is applied to the skin in a medicated ointment), severe refractory uveitis after bone marrow transplants, exacerbations of minimal change disease, Kimura's disease, and the skin condition vitiligo. |

Tacroz Forte Ointment (Tacrolimus) is used to treat atopic dermatitis (more commonly known as eczema). It is a topical medication which is normally prescribed for patients who have not responded successfully to other eczema medication.
The ointment is a type of topical calcineurin inhibitor (TCIs) that works by weakening the patient`s immune system, which as a consequence directly reduces the severity of eczema and its symptoms. It should not be used by patients who have any kind of weak immune system.
The mechanism of action of tacrolimus in atopic dermatitis is not known. the clinical significance of these observations in atopic dermatitis is not known. It has been demonstrated that tacrolimus inhibits T-lymphocyte activation by first binding to an intracellular protein, FKBP-12.
A complex of tacrolimus-FKBP-12, calcium, calmodulin, and calcineurin is then formed and the phosphatase activity of calcineurin is inhibited. This prevents the dephosphorylation and translocation of nuclear factor of activated T-cells (NF-AT), a nuclear component thought to initiate gene transcription for the formation of lymphokines.
Tacrolimus also inhibits the transcription for genes which encode IL-3, IL-4, IL-5, GM-CSF, and TNF-, all of which are involved in the early stages of T-cell activation. Additionally, tacrolimus has been shown to inhibit the release of pre-formed mediators from skin mast cells and basophils, and to downregulate the expression of FceRI on Langerhans cells.
