Indication:
This product is a calcium channel blocker. For the treatment of angina pectoris; hypertension (alone or in combination with other antihypertensive drugs).
Usage and dosage:
Oral administration.
1. The dosage of nifedipine should be adjusted gradually according to patient's tolerance and control of angina pectoris. Overdose of nifedipine can lead to hypotension.
2. Starting from a small dose, the general starting dose is 10 mg/time, three times a day; the commonly used maintenance dose is 10-20 mg/time, three times a day. Some patients with obvious coronary spasm can be used to 20-30 mg/time, 3-4 times a day. The maximum dose should not exceed 120 mg/day. If the patient is in urgent condition, he can take 10 mg chewing or sublingual orally. According to the patient's reaction to the drug, he decides to give it again.
3. It usually takes 7-14 days to adjust the dose. If the patient's symptoms are obvious and the condition is urgent, the dosage adjustment period can be shortened. The dosage of nifedipine can be adjusted from 10-20 mg to 30 mg/time in 3 days according to the patient's response to drugs, frequency of seizures and the dosage of sublingual nitroglycerin.
4. Under strict monitoring, the number of hospitalized patients can be increased by 10 mg every 4-6 hours according to the control of angina pectoris or ischemic arrhythmia.
Side effects
1. Peripheral edema (dose-related, the incidence of 60 mg/day is 4%, 120 mg/day is 12.5%); dizziness; headache; nausea; fatigue and facial flushing (10%). Transient hypotension (5%) often does not need to be stopped (transient hypotension is dose-related, the incidence is 2% when the dose is less than 60 mg / day, and 5% when the dose is 120 mg / day). The occurrence of angina pectoris in some patients may be related to hypotensive response. Palpitation, nasal obstruction, chest tightness, shortness of breath, constipation, diarrhea, gastrointestinal spasm, abdominal distention, skeletal muscle inflammation, joint stiffness, muscle spasm, mental tension, tremor, nervousness, sleep disorder, blurred vision, imbalance, etc. (2%). Syncope (0.5%) did not occur after reduction or combined with other anti-angina drugs.
2. Rare anemia; leukopenia; thrombocytopenia; purpura; allergic hepatitis; gingival hyperplasia; depression; paranoia; blindness at peak blood concentration; erythematous limb pain; anti-nuclear antibody positive arthritis (< 0.5%).
3. Possible serious adverse reactions: incidence of myocardial infarction and congestive heart failure was 4%; incidence of pulmonary edema was 2%; incidence of arrhythmia and conduction block were less than 0.5%.
4. Anaphylactic hepatitis, rash and exfoliative dermatitis may occur in allergic patients.
