Ingredient:Sodium Chloride
Character:
Colorless clear liquid; Taste salty.
Indication:
1. Water loss caused by various reasons, including hypotonic, isotonic and hypertonic water loss.
2. Hyperosmolar nonketotic diabetic coma, the application of isoosmolar or hypoosmolar sodium chloride can correct water loss and hyperosmolar status.
3. Hypochloric metabolic alkalosis.
4. Rinse eyes and wounds with normal saline externally.
5. Also used for inducing labor with water sac in obstetrics department.
Usage:
1. Hypertonic water loss: When hypertonic water loss occurs, the osmotic concentration of brain cells and cerebrospinal fluid in patients increases. If the sodium concentration and osmotic concentration of plasma and extracellular fluid decrease too quickly after treatment, cerebral edema can be caused. Therefore, it is generally believed that the plasma sodium concentration decreases by no more than 0.5mmol/L per hour within 48 hours after the treatment begins. If the patient has shock, sodium chloride injection should be given first, and colloids should be added as appropriate. After the shock is corrected, blood sodium >155mmol/L, plasma osmotic concentration >350mOsm/L, 0.6% hypotonic sodium chloride injection can be given. When the plasma osmotic concentration < 330mOsm/L, 0.9% sodium chloride injection was used. The total amount of rehydration is calculated according to the following formula for reference: required amount of rehydration (L) =[blood sodium concentration (mmol/L)-142]×0.6× weight (Kg) blood sodium concentration (mmol/L) is generally supplemented by half amount on the first day, the remaining amount is supplemented within 2-3 days thereafter, and adjusted according to cardiopulmonary renal function as appropriate.
2. Isotonic water loss: isotonic solution, such as 0.9% sodium chloride injection or compound sodium chloride injection, is given in principle, but the concentration of chlorine in the above solution is significantly higher than that in plasma. Large amount of use alone can cause hyperchloremia, so 0.9% sodium chloride injection, 1.25% sodium bicarbonate or 1.86%(1/6m) sodium lactate can be prepared and supplemented at a ratio of 7:3. The latter has a concentration of 107mmol/L and corrects metabolic acidosis. Supplements may be calculated in terms of body weight or hematocrit for reference.
(1) Calculated by body weight: fluid volume (L)=(weight loss (kg)×142)/154.
(2) According to hematocrit calculation: amount of fluid (L)=(actual hematocrit - normal hematocrit × body weight (kg)×0.2)/ normal hematocrit. Normal hematocrit was 48% in men and 42% in women.
3. Hypotonic water loss:
(1) When severe hypotonic water loss occurs, the solute in brain cells decreases to maintain cell volume. If the treatment makes plasma and extracellular fluid sodium concentration and osmotic concentration rise quickly, can cause brain cell damage. It is generally believed that when blood sodium is lower than 120mmol/L, the rising speed of blood sodium is 0.5mmol/L per hour and no more than 1.5mmol/L per hour.
(2) When the blood sodium is lower than 120mmol/L or central nervous system symptoms occur, 3%-5% sodium chloride injection can be given to relieve the drip. It is generally required to raise the blood sodium concentration to more than 120mmol/L within 6 hours. Sodium supplement amount (mmol/L) = [142- actual blood sodium concentration (mmol/L)]× body weight (kg)×0.2. When the blood sodium rises to more than 120-125mmol/L, hypertonic glucose injection or 10% sodium chloride injection can be added to the isosmotic solution or isosmotic solution as appropriate.
4. Give 0.9% sodium chloride injection or compound sodium chloride injection (Ringer's solution) 500-1000ml for low-chlorine alkalosis, and then decide the dosage according to the situation of alkalosis.
5. For external use, wash the wound and eyes with physiological sodium chloride solution.
Adverse reactions:
1, too fast, too much can lead to high blood pressure, headache, dizziness.
2. Weight gain and edema.
3. Increased heart rate, chest tightness, dyspnea and wheezing in the lungs.
Taboo:
The following patients are prohibited:
1. Heart failure.
2. Pulmonary edema.
3. Cerebral edema and increased intracranial pressure.
4, cirrhosis ascites.
5. In the oliguria stage of acute renal failure, chronic renal failure does not respond well to diuretics.
5. Hypernatremia.
Attention:
1. The following situations should be used with caution:
(1) Edema due to pregnancy.
(2) Hypertension.
(3) Brain edema, edema or edema tendency. Hyperedema with hyponatremia should be noted.
(4) Mild heart and renal insufficiency.
(5) Hypokalemia.
(6) Ascites with cirrhosis.
(7) The drug should be administered according to the properties of hypertonic, isotonic or hypotonic water loss, and other solutions should be considered to maintain the balance between various electrolytes in the body.
(8) Follow up to check serum potassium, sodium, chlorine concentration, acid-base balance, cardiopulmonary kidney function, blood pressure and other indicators.
(9) Check carefully before use. Do not use if the solution is turbid, foreign matter, bottle cap is loose or cracked.
(10) The molality concentration of osmotic pressure of this product is 260-320m0SMOL /kg.
2. Drug overdose: can cause hyperkalemia and hypokalemia, and can cause bicarbonate loss, excessive and too fast infusion volume, can lead to water and sodium retention, causing edema, increased blood pressure, accelerated heart rate, chest tightness, dyspnea, and even acute left heart failure.
Drug interaction:
When used as a solvent or diluent, drug compatibility contraindications should be noted.
1. When combined with amphotericin B, there is turbidity, precipitation and discoloration.
2, the forbidden and reserpine, the polymyxin B sulphate, polymyxin E sulfate, cephalothin I compatibility.
