Diclofenac sodium is a common nonsteroidal anti-inflammatory drug that can relieve acute or persistent joint swelling and pain in rheumatoid arthritis, rheumatoid arthritis, osteoarthritis, gout arthritis and other types of arthritis. Soft tissue pain, such as shoulder pain, tenosynovitis, bursitis, myalgia and post-exercise injury pain. Acute mild or moderate pain such as surgery, trauma, pain after strain, primary dyspenorrhea, toothache, headache, etc. In general, diclofenac sodium has a very good anti-inflammatory and analgesic effect.
Diclofenac sodium is a white or quasi-white, odorless, hygroscopic crystalline powder, which reduces the synthesis of prostaglandins by inhibiting cyclooxygenase. Commonly used sodium salt, colorless crystal, odorless, soluble in water, ethanol and other polar organic solvent, aqueous pH=7.68, pKa=4. Melting point of 283-285 ºC.
Diclofenac is a non-steroidal anti-inflammatory and analgesic drug derived from phenylacetic acid. Its mechanism of action is to inhibit the activity of cyclooxygenase, thus blocking the transformation of aracidonic acid into prostaglandin. It also promotes the binding of arachiidonic acid to triglycerides (triacylglycerols), reduces the concentration of free arachiidonic acid in cells, and indirectly inhibits leukotrienes synthesis.
Analgesic effect: mainly by inhibiting prostaglandin and other substances that can make pain sense sensitive to mechanical or chemical stimuli (such as bradykinin, histamine), it is a peripheral analgesic drug. However, the possibility of central analgesia (which may affect the hypothalamus) cannot be ruled out.
Anti-inflammatory effect: The exact mechanism is not yet clear. It may be due to the effect of this product on inflammatory tissue, inhibiting the synthesis of prostaglandin or other substances that can cause inflammatory response (such as histamine), and it may also be related to the inhibition of lysosomal enzyme release and leukocyte activity.
Antipyretic effect: the antipyretic effect may be caused by the action on the hypothalamus thermoregulation center to cause peripheral vascular dilation, skin blood flow increase sweating and heat dissipation increase, and this central effect may be related to the inhibition of the synthesis of prostaglandin in the hypothalamus
Anti-rheumatism: the anti-rheumatism mechanism of the product, in addition to antipyretic, analgesic effect, mainly lies in the anti-inflammatory effect;
Inhibition of platelet aggregation: It acts by inhibiting prostaglandin cyclooxygenase, which prevents the production of Thromboxane A2TXA2 (TXA2 promotes platelet aggregation). This effect is irreversible.
Diclofenac sodium is one of the stronger non-steroidal anti-inflammatory drugs, and its inhibitory effect on prostaglandin synthesis is stronger than that of aspirin and indometacin. Non-clinical toxicological studies: Rats were given diclofenac sodium orally at 2mg/kg daily, and no increase in tumor incidence was found after long-term observation. A two-year study of mice administering 2mg/kg daily also did not show any tumor predisposition.
