Prostaglandin E1 PGE1 exerts a number of effects, including systemic vasodilation, inhibition of platelet aggregation, and stimulation of intestinal motility. PGE1 relaxes isolated smooth muscle cells contracted by norepinephrine. It has become widely used in the treatment of ED. Alprostadil binds with PGE receptors and results in a cyclic adenosine monophosphate (cAMP) mediated smooth muscle relaxation. Little is known about the pharmacokinetics of PGE1, but it is believed that as much as 80% is metabolized in one pass through the lungs. Such rapid degradation probably accounts for its lack of significant cardiovascular side effects when administered intracavernosally. PGE1 can also be metabolized in the penis.

Function

Prostaglandin E1 is used to treat erectile dysfunction through its vasodilatation function and is sold under the American trade name Muse or Vivus for erectile urethral embolization. It can also be used as a brand name for Caverject and Edex, which are injectable forms. Prostaglandin E1 will be used as a penile drug delivery system to insert the urethra for at least ten minutes before you need an erection. In contrast, the same quick effect of Caverject and Edex is also directly injected into the penile corpus cavernosum.
The drug is recommended for men with erectile dysfunction. You can also use this drug to keep the patent ductus arteriosus of the newborn. This is primarily effective when ductal ductus arteriosus is not associated with ductal congenital heart disease, including areola lesions (ie, aortic arch disruption, aortic stenosis, dysplastic left ventricular syndrome and aortic coarctation). The Prostaglandin E1 is used for penile erection as needed. The effect starts at about 5 to 10 minutes after administration and lasts for about 30 minutes to 1 hour.
PGE1 is an effective drug for opening the catheter to a catheter that relies on congenital heart disease. It can be used in the first week of life than the efficacy of newborns. Apnea is the main side effect, and close monitoring is essential.

Application

Prostglandin E1 is used to treat erectile dysfunction (ED) in adult males. The efficacy of alprostadil in treating ED varies with the cause; response rate is generally lower in patients with ED due to mixed etiologies compared to those with ED due to neurogenic, psychogenic, or vasculogenic causes. Additionally, PGE1 has been shown to affect protein kinase C (PKC), calcium movement, and adenylate cyclase yielding a multitude of physiological effects.

Prostaglandin E1 will prevent the ductus arteriosus from closing and reestablish ductal patency if closure has already occurred, and thereby increase PaO2, and mitigate the onset of metabolic acidosis. This drug is indicated for the temporary management of the neonate with ductus-dependent congenital heart disease while awaiting transfer to a tertiary care nursery for evaluation and surgical therapy. It is also used to stabilize a neonate's condition until surgery can be completed.
