Oseltamivir is a prodrug of its active metabolite, and its active metabolite (oseltamivir carboxylate) is a selective influenza virus neuraminidase inhibitor. Neuraminidase is a glycoprotein on the surface of the virus whose activity is essential for the release of newly formed viral particles from infected cells and the further spread of infectious viruses in the human body.

The active metabolite of oseltamivir phosphate inhibits the neuraminidase activity of influenza A and B viruses. The half-inhibitory concentration of viral neuraminidase activity in vitro is as low as the nanogram level. It has been observed in vitro that active metabolites inhibit the growth of influenza virus, and its inhibition of influenza virus replication and pathogenicity is also observed in vivo. This product reduces the spread of influenza A or B by inhibiting the release of the virus from infected cells.

Studies of naturally acquired and laboratory influenza have shown that the use of oseltamivir phosphate does not affect the body's normal humoral immune response to infection. The antibody response to the inactivated vaccine was not affected by treatment with oseltamivir phosphate.
